The County of Orange Health Care Agency has asked for our help in conducting a survey of attitudes toward vaccination for COVID-19, which will be of great assistance to them in planning for distribution of the vaccine when it becomes available. We would like them to listen to your opinion and we are asking you to take less than five minutes to fill out a simple questionnaire concerning how you feel about being vaccinated against COVID-19.
